CDC's Native Diabetes Wellness Program
is currently collecting Shared Data Elements from Tribes and Tribal
Organizations. Awardees are building on traditional ways to inform
culturally relevant approaches to diabetes prevention.

Respondents are cooperative
agreement awardees. The funding period ends 9/30/2014 and the
current OMB clearance period ends 6/30/2014. A short (apx. 6 month)
extension is needed so that CDC can receive final SDE reports from
awardees. There are no changes to the data collection instrument or
the estimated burden per response. Information has been collected
twice per year, however, only one report will be due during the
period of this Revision request. The number of respondents will
decrease from 17 to 16. These changes result in a reduction in
total burden.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
