Proposed changes to CPS questionnaire


  1. Proposed CPS Certification/Licensure Questions

  1. (Does/Do) (name/you) have a currently active professional certification or a state or industry license? Do not include business licenses, such as a liquor license or vending license.


Asked of everyone (months 1 and 5 of 8-month rotation)


(Read if necessary: A professional certification or license shows you are qualified to perform a specific job. Examples include a realtor license, a medical assistant certification, a Teacher’s License, or an IT certification. Only include certifications or licenses obtained by an individual.)
