Your organization's data submission timeline began on [Organization effective date]. Your scheduled data submission, due on [Anniversary of Org Effective Date], is now [# of weeks overdue, minimum 4] weeks overdue. Per the DPRP Standards, recognized organizations are required to submit evaluation data every 12 months. Failure to submit data as required will result in loss of recognition and removal from the DPRP Registry.

The purpose of this email is to inform you that your organization has lost recognition, due to not submitting the required data, and will be deleted from the CDC DPRP Registry of Recognized Organizations.

If you have submitted the required data and believe this notification has been sent in error, please contact us as soon as possible at [email protected] so that we can clear up any miscommunication. The subject line of your email must include your organization name and your assigned DPRP organization code.
