Preexposure Prophylaxis (PrEP): Local health Department Assessment Instrument
The National Association of County and City Health Officials (NACCHO), with funding and support from
the Centers for Disease Control and Prevention (CDC), is conducting this assessment to establish a better
understanding of local health department's (LHD's) HIV preexposure prophylaxis (PrEP) perceived role in
implementation activities, needs, challenges, opportunities, and next steps. This information will allow
NACCHO, as well as its governmental and nongovernmental partners at the national, state, and local level,
to better support and promote LHD engagement in PrEP implementation.
The assessment should take approximately 30 minutes to complete. Once you begin the assessment, you
will be able to save and continue at a later time. To preview the assessment, you can access a PDF version
of the questions. Please consult with other staff members informed about your health department’s PrEP
activities, as needed, to complete the assessment.
Thank you in advance for your time and attention to this assessment. If you have any questions, please
contact Gretchen Weiss, Senior Program Analyst for HIV/STI/HCV at [email protected] or 202507
4276.
Public reporting burden of this collection of information is estimated to average 30 minutes per response,
including time for reviewing instructions, searching existing data sources, gathering and maintaining the
data needed, and completing and reviewing information. An agency may not conduct or sponsor, and a
person is not required to respond to a collection of information unless it displays a currently valid Office
of Management and Budget control number. Send comments regarding this burden estimate, or any other
aspect of this information collection, including suggestions for reducing this burden to CDC/Agency for
Toxic Substance and Disease Registry Information Collection Review Office, 1600 Clifton Road NE, MS
D74, Atlanta, Georgia 30333; Attention: PRA (09200879).
