Instructions for Local Health Departments Serving Multiple Counties

We have identified your local health department as serving more than one county.

Enclosed in this mailing are supplemental questions for each of the counties covered by your local health department. These supplements only include questions on policy development and implementation specific to each county your local health department serves.

You can choose to complete one assessment on behalf of all the counties served by your local health department, or use the supplements to answer the questions on policy development and implementation separately for each county.

There is space on page 1 of each supplement to enter the county for which you are responding.

If you are not the most knowledgeable person to respond for each county, we encourage you to pass the supplements out to appropriate staff who can answer the questions on policy development and implementation for each county. That person may be a department head or chronic disease director.
