Attachment J – State Level Phone Interview Reminder Email

Subject: Request to participate in School Health Branch Assessment

Body

Dear ,

We are following up with an additional appeal to you to complete a 50-minute phone interview to learn how you are using tools and resources that the Centers for Disease Control and Prevention has created to create healthier school environments and programs. We are working on this project in partnership with Sarah Lee with the CDC’s School Health Branch and with funding from the CDC. You have been selected to be interviewed because you coordinate your state’s school health work within the “State Public Health Actions to Prevent and Control Diabetes, Heart Disease, Obesity, and Associated Risk Factors and Promote School Health (1305 cooperative agreement).” Your input is vital providing us the input we need to improve the CDC’s school health tools and resources (listed at end of email).

We ask that you please respond by DATE.

Please do not hesitate to contact us with any questions, and thank you again for your support of this project.

Sincerely,

Jennifer Leeman, DrPH
