National Syndromic Surveillance Program
Script for announcing Success Stories From the Field Template
Attachment D
Script: NSSP awardees
Venue: Syndromic Surveillance webinars; NSSP awardee monthly and quarterly meetings and site visits; and Annual NSSP Grantee Meeting
CDC’s Center for Surveillance, Epidemiology and Laboratory Services (CSELS), Division of Health Informatics and Surveillance (DHIS), wants to know how you have implemented best practices into the National Syndromic Surveillance Program (NSSP). These stories will showcase your experience and help others implement NSSP.
CSELS has developed a template to assist you in preparing your success story. The form can be accessed online at https://www.cdc.gov/nssp/success-stories.html. Each section of the form is followed by a few self-check questions to help you address relevant benchmarks.
The upper limit of time to complete this form including time for reviewing instructions, gathering needed information and completing the form is 100 minutes, with an average of 83 minutes.
This tool will be used by NSSP cooperative agreement awardees who are required to elaborate on their NSSP successes annually. The form will be incorporated into the NSSP FOA guidance and introduced and distributed during syndromic surveillance webinars; the NSSP awardee monthly/quarterly meetings and site visits, and the annual NSSP Grantee Meeting.
Completed success stories can be emailed to CDC’s National Syndromic Surveillance Program at [email protected].
