Good afternoon, my name is [Name] with [CDC/HRSA/CMS]. We are conducting an assessment of the HIV Health Improvement Affinity Group or HHIAG. I am calling to see if you would be willing to participate in this assessment. Your participation would involve a 60-minute phone interview to help us understand (1) the extent to which, and how, participation in the HHIAG was useful for developing and implementing an action plan to improve health outcomes for persons living with HIV that are enrolled in Medicaid/CHIP and (2) what were the lessons learned from implementing the action plans including successes, challenges, and promising practices.
