The majority of the comments requested more clarification on how to use the form. Specifically, commenters were unsure of what would constitute a completed form, who specifically must sign the it and whether the form was required to be used. As a result we have expanded the form’s instructions to address questions raised. Some commenters suggested changes in some of the terms used. We considered each comment and made changes where appropriate (see Crosswalk).
