Large Format: For use on larger printed materials such as flyers, handouts, postcards, posters, etc. For materials promising incentives for completion of the survey, the amounts will be either $10 or $20.

Complete a brief anonymous survey online to improve roadway safety

and you will receive a $ 10 gift card

As part of a national research project, drivers 18 years of age or older are being asked to complete a brief and anonymous online survey about driving behaviors. This survey is completely anonymousno personally-identifying information (such as your name) will be requested, tracked, or reported. Your answers will only be used as part of a large sample and will only be accessed by the research team conducting this study. Your state department of motor vehicles will NOT have access to any of your survey answers.

The survey takes about 12 minutes to complete. The data collected by the survey from thousands of drivers in the U.S. (known as aggregate data) will be used by researchers to understand the link between driving incidents and certain types of driving behaviors. Upon completing the survey you are guaranteed to receive a code to enter at the National Gift Card Company for a $10 gift card. You can you pick from among more than 70 choices of stores and restaurants for your $10 gift card.
