T ITLE OF INFORMATION COLLECTION: NASA Emergency Notification and Accountability System (ENS) Survey Following a Drill or Event.

PURPOSE: NASA is developing a survey for release following a drill or actual event as part of its emergency preparedness program. Per this information collection, NASA will test the NASA Emergency Notification and Accountability System (ENS) survey for clarity and usability prior to finalization.

The ENS system will release either an email or automated phone call to targeted respondents.

When the automated phone call approach is used, the recipients will hear the same message/content that is provided via email.

DESCRIPTION OF RESPONDENTS: NASA contractors whose duty station is within NASA security perimeters and/or leased space.

TITLE OF INFORMATION COLLECTION: Provide the name of the collection that is the subject of the request. (e.g. Comment card for soliciting feedback on xxxx)

PURPOSE: Provide a brief description of the purpose of this collection and how it will be used. If this is part of a larger study or effort, please include this in your explanation.
