T ITLE OF INFORMATION COLLECTION: Health and Safety Fair Customer Feedback Survey


PURPOSE:

NASA Headquarters promotes an annual Health and Safety Fair and NASA HQ, run by the Facilities and Emergency Services Division (FESD), and on-site contractors are encouraged to participate. This year’s fair will take place on September 13, 2018. The purpose of the survey is gather feedback from participants to improve future events.



DESCRIPTION OF RESPONDENTS:


NASA Headquarters contractors (private sector) attending the annual Health and Safety Fair.

FEDERAL COST: The estimated annual cost to the Federal government is: $33.33

The way we calculated this number was based on an estimated 50 private sector respondents spending approximately 1 minute on the survey for a burden of 50 minutes (or 5/6 of an hour), then multiplying that by $40.00 (the number we have found for the average Federal wage) to come to $33.33.

The universe of potential respondents will be the sample of employees who attend the Health and Safety Fair. The respondents will be random and selected by voluntary willingness to complete the survey. While hundreds of employees attend the Health and Safety Fair, a minimal number of employees historically complete the event surveys. In 2017, extra volunteers were available who could assist with promoting the survey, and total surveys received was 79. In the three years prior, volunteers were not as prevalent, and there were 15, 38, and 29 total surveys completed respectively for previous years. An average of the four recent years is 40. This year, the survey will be administered digitally via iPads, as opposed to previous written surveys, and the event planning team projects better participation due to greater ease of application format.

TITLE OF INFORMATION COLLECTION: Provide the name of the collection that is the subject of the request. (e.g. Comment card for soliciting feedback on xxxx)


PURPOSE: Provide a brief description of the purpose of this collection and how it will be used. If this is part of a larger study or effort, please include this in your explanation.


DESCRIPTION OF RESPONDENTS: Provide a brief description of the targeted group or groups for this collection of information. These groups must have experience with the program.


TYPE OF COLLECTION: Check one box. If you are requesting approval of other instruments under the generic, you must complete a form for each instrument.


CERTIFICATION: Please read the certification carefully. If you incorrectly certify, the collection will be returned as improperly submitted or it will be disapproved.


Personally Identifiable Information: Provide answers to the questions. Note: Agencies should only collect PII to the extent necessary, and they should only retain PII for the period of time that is necessary to achieve a specific objective.
