1. Introduction
Thank you for taking the time to provide feedback on your recent animal health emergency response
assignment. Your feedback is valuable and will contribute to APHIS VS’s efforts to improve our
animal health emergency response capabilities.
This survey is completely anonymous and should take the average responder approximately 30
minutes to complete. You will only be asked to evaluate activities that were applicable to your
response role. Please answer all of the questions as completely as possible. You are encouraged to
provide recommendations, even for things that worked well. It is critical that you provide ideas and
suggestions for areas that you believe need improvement.
