NHSN is a system designed to accumulate, exchange, and integrate relevant information and resources among private and public stakeholders to support local and national efforts to protect patients and promote healthcare safety.The NHSN has six components: Patient Safety, Healthcare Personnel Safety, Biovigilance,Long-Term Care Facility, Dialysis, and Outpatient Procedure. The data are used to determine the magnitude of various healthcare-associated adverse events and trends in the rates of these events among patients and healthcare workers with similar risks,and to detect changes in the epidemiology of adverse events resulting from new and current medical therapies and changing risks. As of March, 2015, 33 states and the District of Columbia have opted to use NHSN as the operational system for mandatory reporting by healthcare facilities in their jurisdictions, and additional states are expected to follow with similar use of NHSN for mandatory reporting purposes. In addition, the U.S. Centers for Medicare and Medicaid Services (CMS) requires healthcare facilities to report HAI and prevention practice adherence data via NHSN in fulfillment of CMSâs quality measurement reporting requirements for those data. Reporting institutions will be able to access their own data at any time and analyze them through the internet interface. Twenty-seven previously approved forms were updated based on user feedback and clarifications in surveillance definitions. Two forms were removed from the package as they were not built into NHSN. Attachment G-1(Crosswalk for Supporting Documentation and Corresponding Data Collection Forms) will provide a table indicating which documents supports what data collection instrument under Attachment C.
