CDC Burden of Canine Brucellosis Information Collection Telephone Script

This script will only be used if the response rate is below 30% at one monthto call all non-responders three weeks after the requested due date, which is seven weeks after the distribution of the assessment. The script will be delivered by the Brucellosis Epidemiologist or an unpaid student.

Good morning/afternoon.

I am calling from the Centers for Disease Control and Prevention in Atlanta, GA regarding the Burden of Canine Brucellosis Information Collection. We have not yet received a completed submission from your laboratory. The collection is voluntary, though we hope that you will consider participating, as your responses will inform the planning of future public health research of Brucella canis human infections.

If you choose to complete it, please submit the information electronically in the next two weeks.
