Thank you for your willingness to participate in this survey. Your participation is very important to the U.S. Small Business Administration (SBA). The information collected from you and other ScaleUp participants through the baseline survey and post-participation surveys will be used to evaluate the performance of the ScaleUp initiaitive.

Your completion of the survey is extremely valuable because the information you provide each year will help SBA assess the initiative’s impact on business growth overtime and improve the services for future small business participants.

SBA has contracted Optimal Solutions Group, LLC (Optimal) to conduct this independent evaluation of the initiative and to administer this survey. The survey will take approximately 20 minutes to complete. You can stop the survey and start a new session by clicking the link again at any time during the survey period. Your previous answers will be saved, allowing you to resume the survey where you stopped.
