Transportation and Health Tool (THT) Website User Satisfaction Assessment

Attachment 1- Recruitment Email for On-line User Satisfaction Survey

Potential survey participants will receive a personalized e-mail invitation containing the link to the survey:

Dear [First Name]:

You’re being asked to participate in an online survey about the Transportation and Health Tool website developed by the Centers for Disease Control and Prevention and US Department of Transportation. We are contacting you because you registered for a webinar about the Transportation and Health Tool in the past year.

I work for Shattuck and Associates, an independent program planning and evaluation company. We are working with the American Public Health Association on an evaluation of the Transportation and Health Tool. We hope to learn about user’s satisfaction and experiences with the Transportation and Health Tool. The information you provide will be used to help improve the tool.

The survey should take about up to 10 minutes to complete. It will be available until [Survey Close Date]. Your participation in the survey is completely voluntary and no personally identifiable information will be collected. We request that you complete the survey even if you have not yet visited the Transportation and Health Tool website so that you can tell us why.

If you agree to participate, please click HERE [link] to begin the survey.

If you have any questions about the survey, please contact me at the e-mail address below.

Thank you,

Sandy Saperstein, PhD
