C onnectHome is a pioneering program in your community that helps children access the Internet at home to study, do homework, and communicate with their teachers.

W e’re asking you to complete a brief 2 to 5 minute survey so we can make the best program possible:

• Please fill out the attached survey form and mail it back in the enclosed pre-addressed envelope—you do not need to add a stamp.

• We will give $5 cash to 200 of the people who answer and return the survey.

This survey is voluntary. You do not have to participate and you can decide not to answer any specific questions. Moreover, taking part in this survey will not affect your housing benefits. Please also be aware that the information you provide will be kept confidential to the extent permitted by law and that folks in your community may reach out to you to encourage you to complete the survey.
