Mathematica Policy Research is conducting an important survey, sponsored by the U.S. Department of Labor (DOL), of people who applied to the Self-Employment Training (SET) Demonstration program. You have been identified from lists of people who applied to the program in your area. About [XX]twelve months ago, you received some information about this study and agreed to participate in it. We told you that we would contact you again in about twelve months. We would like to hear about your experiences with self-employment and self-employment assistance services, wage or salary employment, and your overall well-being over the past [XX] twelve months. We will be conducting an online survey. Below we provide information on how to access the questionnaire online. If you do not complete the survey online, a Mathematica interviewer will contact you to offer to complete the survey by telephone. The survey takes about 60 minutes to complete.
