OSHA is requesting a non-substantive change to the currently approved collection of information requirements contained in the “Recordkeeping and Reporting Occupational Injuries and Illnesses (29 CFR Part 1904)” ICR to add a [email protected] web mail box to the Serious Event Reporting Online form.


29 CFR 1904.39(a) requires employers of any employees so affected to orally report fatalities and hospitalization by telephone or in person to the nearest OSHA Area Office that is nearest to the site of the incident, or by using the OSHA toll-free central telephone number. Recent revisions to the requirements in §1904.39 allow employers to develop and submit the report electronically using OSHA’s public website.


As required by PRA, the PRA burden box on Serious Event Reporting Online form informs the public that they may comment on of the Form and provides a mailing address for them to submit their comments. OSHA recognizes that the public should be able to submit comments electronically to the Agency. OSHA proposes to add an electronic mailbox [email protected] to the burden box on the form to allow the public the option of submitting comments to OSHA electronically.
