Re: DP15-1502 CRCCP Clinic Level Information Collection
Your annual submission of CRCCP Clinic-level information is due no later than [actual date TBD pending OMB approval]. CRCCP grantees are required to report a set of standardized data each year to CDC on all partner health systems and clinics where CRCCP activities are planned and implemented. Standardized data have been previously defined and include descriptive information on each participating health system, the development status of evidence-based interventions and supporting activities implemented through the CRCCP to increase colorectal cancer screening within each partner health system, and an annual CRC screening rate for each participating health system. Your information will be used to monitor program activities and assess outcomes.
The CRCCP clinic-level data entry system is accessed from the program website, www.crccp.org, Clinic tab. If you have questions about the content, please contact your CDC Program Consultant.
