PRA Disclosure Statements

CMS-367a, -367b, and -367c

The CMS-367a, -367b, and -367c screen shot file that was submitted to OMB on March 21, 2016, contains several errors specific to the PRA Disclosure Statement. Specifically, the Statements for the CMS-367a and -367b screen shots are missing while the statement for CMS-367c incorrectly sets out 15.8 hours.

Consequently, CMS will be making the following three (3) corrections:

1-The PRA Disclosure Statement will be added to the CMS-367a form. The revised screen shot will set out 34.8 hours (per response) and will be submitted to OMB as a nonsubstantive change by March 31, 2016.

The Statement will read as follows:

According to the Paperwork Reduction Act of 1995, no persons are required to respond to a collection of information unless it displays a valid OMB control number.  The valid OMB control number for this information collection is 0938-0578.  The time required to complete this information collection is estimated to average 34.8 hours per response, including the time to review instructions, search existing data resources, gather the data needed, and complete and review the information collection.  If you have comments concerning the accuracy of the time estimate(s) or suggestions for improving this form, please write to: CMS, 7500 Security Boulevard, Attn: PRA Reports Clearance Officer, Mail Stop C4-26-05, Baltimore, Maryland 21244-1850.
