Purpose of this Submission: This submission is being made for proposed non-substantive changes to an existing information collection pursuant to 44 U.S.C. § 3507. This submission seeks to update: (i) the FCC Form 498; (ii) the accompanying instructions to the FCC Form 498.
Summary of Proposed Updates to the FCC Form 498:
Spelling Correction: Block 3 on line 22 of the FCC Form 498 (page 1) the spelling of the word "Dun has been corrected to “Dunn.”
Summary of Proposed Updates to the FCC Form 498 Instructions:
Validate Banking Information: As described in Appendix B, additional information may be requested to validate banking and routing information (i.e., copy of bank statement and/or voided check).
