Non-Substantive Change Request 0584-0474

Through this memorandum, we are requesting a non-substantive change to the Summer
Feeding - National Hunger Clearinghouse Excel Form, FNS-543A, OMB Control No.
0584-0474; expiration date October 31, 2018.

Two columns of the existing form were inadvertently dropped in the prior submission of
form FNS-543A. Additionally, form FNS-543A is an excel form and the form was
submitted as a data sheet. This request corrects the prior errors. With this correction, we
are requesting a revision to Column Q of the previously cleared excel form. Column Q
currently reads “Hours Open.” FNS is proposing to revise the field to “Hours or
Additional Information.” Sites have requested a place to provide additional information
for summer feeding sites. For example, if a summer feeding site is located in a park, they
want to indicate where or if the site is in a school, they may want to indicate which door to
enter. There is no change in burden requested.
