The Creditable Coverage Disclosure Form has been updated to allow the form to fit on one page, instead of the previous multiple pages. None of the data elements and/or information has changed.
This nonsubstantive change will provide permit entities with information pertaining to the new contractor and will furnish the more efficient form.
