Rationale for the Change
CMS 64.9VIII Enrollment Form: The form contains all VIII Group
eligible. It contains eligible categories and 4 quarters. Only one
quarter can be filled at a time. The quarter will be moved
forward quarter by quarter.
The form contains all eligible categories of Group
64.9VIII. Added few forms as the result of the
implementation of Section 2001(a)(1). It covers
individuals with income at or below 133% of FPL
To comply with the Affordable Care Act (ACA) Section 2001 (a)(1). It
also comply with the Social Security Act (SSA) Section
1902(a)(10)(A)(i) adding Group VIII beginning January 1, 2014.
CMS 64.9VIII Expenditure Form-Not Newly: Not-Newly form is
available for each eligibility groups for the State, such as
parent/Care taker Relatives, disabled person in instutionalzed
or non-institutionlzed, Children Age 19 to 20, Childless Adult
etc. Not Newly forms provide functionality to transfer funds to
the Newly form based on Resource test, enrollment Cap and
Special Circumstances authorized to each State.
These forms provide expenditure reporting for
Medical Assistance Payments for CMS VIII groups.
To comply with the Affordable Care Act (ACA) Section 2001 (a)(1). It
also comply with the Social Security Act (SSA) Section
1902(a)(10)(A)(i) adding Group VIII beginning January 1, 2014.
CMS 64.9VIII Newly Expenditure Form: CMS 64.9VIII Newly
eligible form allows entry of expenditures related to Newly
eligible groups to be with Federal Funds calculated at Newly
eligible rate defined by the law. Additionally, the form reports
funds transferred from the Not Newly forms for eligible groups,
which are funded at the Newly Rate (100%).
The form provides expenditure reporting for
Medical Assistance Payments for VIII eligible
groups. A waiver version of these forms also
available for waiver programs.
To comply with the Affordable Care Act (ACA) Section 2001 (a)(1). It
also comply with the Social Security Act (SSA) Section
1902(a)(10)(A)(i) adding Group VIII beginning January 1, 2014.
Similarly, CMS 64.9VIII Form for Waivers: Newly and Not Newly

forms are available for waivers for qualified States as of 2
quarter of 2014.
The form provides expenditure reporting for
Medical Assistance Payments for VIII eligible
groups. A waiver version of these forms also
available for waiver programs.
To comply with the Affordable Care Act (ACA) Section 2001 (a)(1). It
also comply with the Social Security Act (SSA) Section
1902(a)(10)(A)(i) adding Group VIII beginning January 1, 2014.
Starting 3rd quarter of 2014, 64.9VIII P forms are also
being available for qualified States for eligibility groups
and waivers to report prior period adjustments.
The form provides expenditure reporting for
Medical Assistance Payments for VIII eligible
groups. A waiver version of these forms also
available for waiver programs.

As a result of these new forms, the following Forms have been
updated: CMS 64 Summary From, FMR and Expenditure
Reports, CMS 64.9A, CMS 64.9c1, CMS 64.9O, CMS 64.9OFWA,
CMS 64.9OPerm, CMS 64.9ORAC and CMS 64.9SRAC to update
expenditures of 64.9VIII.

groups. A waiver version of these forms also
available for waiver programs.
