submitted on occasion by state member banks (SMBs), bank holding
companies (BHCs), savings and loan holding companies (SLHCs), and
foreign dealer banks that are municipal securities dealers. The
Form MSD-4 collects information (such as personal history and
professional qualifications) on an employee whom the bank wishes to
assume the duties of municipal securities principal or
representative.
