Individualized Quality Control Plan (IQCP) Survey
In May 2015, the Centers for Disease Control and Prevention (CDC) in collaboration
with the Centers for Medicare & Medicaid Services (CMS) released the IQCP-A StepBy-Step Guide, which is available both in print and electronically at
https://wwwn.cdc.gov/clia/Documents/IQCP Layout.pdf. This workbook provides a
voluntary guide to assist laboratories in developing IQCP, which is an alternate quality
control (QC) option under the Clinical Laboratory Improvement Amendments (CLIA)
regulations for most nonwaived testing. The IQCP option offers a laboratory the
flexibility for meeting regulatory QC requirements appropriate for the testing performed,
including existing and new test systems.
The CDC Division of Laboratory Systems (DLS) seeks to examine the utility of the IQCP
workbook and its components in assisting laboratories with evaluating the quality of
their current testing procedures and developing an IQCP that meets the CLIA
requirements.
Please complete this brief 10 minute survey about your experience with the IQCP
Workbook.
Public reporting burden of this collection of information is estimated to average 10 minutes per
response, including the time for reviewing instructions, searching existing data sources,
gathering and maintaining the data needed, and completing and reviewing the collection of
information. An agency may not conduct or sponsor, and a person is not required to respond to
a collection of information unless it displays a currently valid OMB control number. Send
comments regarding this burden estimate or any other aspect of this collection of information,
including suggestions for reducing this burden to CDC/ATSDR Reports Clearance Officer; 1600
