The Community Guide is a free resource to help choose
evidence-based interventions that could be used to improve
health and prevent disease in your community. The purpose of
this survey is to gather baseline data to assess your level of
awareness and use of The Community Guide. Please do not
forward this email and only complete this survey
once. The survey should take no longer than 15 minutes.

The public reporting burden of this collection of information is estimated to average 15 minutes per
response, including the time for reviewing instructions, searching existing data sources, gathering and
