Invitation Email for Local Users

Dear Local User,

The NSSP Team at the Centers for Disease Control and Prevention (CDC) is conducting a survey entitled “The BioSense Platform User Satisfaction Survey for Local Users” in conjunction with the BioSense Platform User Satisfaction Survey for Site Administrators.

As a site administrator, I have been requested by the NSSP Team at the CDC to invite a local user to complete the BioSense Platform User Satisfaction Survey for Local Users. In response to this request, I kindly ask you to complete the survey. Please note that participation in this survey is voluntary. The information you provide will help the NSSP team better understand if the tools are functioning as intended for users to conduct their syndromic surveillance activities, if the tools are meeting user needs and if there are any improvements necessary.

Your feedback is important to improving the BioSense Platform. Please complete the survey that you can access by using the Epi Info web-based link below (for more information on Epi Info, please visit http://www.cdc.gov/epiinfo/index.html).

The survey will take approximately 10 minutes to complete. You also have the option to finish the survey at a later time by clicking on the “Finish Later” button which will bring you to a dialog box with a survey link and passcode. You will have the options either to copy and save the survey link and passcode or to enter your email address and receive the survey link and passcode in an email. Your email address will not be saved in the system. To reopen your survey, copy and paste the survey link into the browser or click the link in the email sent to you. Enter the passcode and continue where you left off previously. This passcode is very important to access your prior saved answers. Please make sure to answer all questions you are able to.
