Subject: BioSense Platform User Satisfaction Survey for Site Administrators and BioSense Platform User Satisfaction Survey for Local Users

Dear Site Administrators,

Welcome to the BioSense Platform User Satisfaction Survey for Site Administrators and the BioSense Platform User Satisfaction Survey for Local Users. The information you provide will help the NSSP team better understand if the tools are functioning as intended for users to conduct their syndromic surveillance activities, if the tools are meeting user needs and if there are any improvements that are needed.

Your feedback is important for helping us improve the BioSense Platform. We kindly ask you to complete the BioSense Platform User Satisfaction Survey for Site Administrators that you can access by using the Epi Info web-based link below (for more information on Epi Info, please visit http://www.cdc.gov/epiinfo/index.html). Please note that participation in this survey is voluntary.

It will take approximately 15 minutes to complete. You also have the option to finish the survey at a later time by clicking on the “Finish Later” button which will bring you to a dialog box with a survey link and passcode. You will have the options either to copy and save the survey link and passcode or to enter your email address and receive the survey link and passcode in an email. Your email address will not be saved in the system and will only be used to send you the survey link. To reopen your survey, copy and paste the survey link into the browser or click the link in the email sent to you. Enter the passcode and continue where you left off previously. This passcode is very important to access your prior saved answers. Please make sure to answer all questions you are able to.

Additionally, please send an invitation email to a local user in your jurisdiction to complete the BioSense Platform User Satisfaction Survey for Local Users. The local user needs to be someone who regularly uses the BioSense Platform to conduct activities and accessed the BioSense Platform in the past 30 days. Please use the draft email and survey link included in the attachment.
