The screen shots have been divided into two sections.
1. For Emergency Workers: All files in this folder contain screen shots from the
iPhone app that are intended for the Emergency/Relief workers providing data on
“found’ people. The folder contains the following files.

iPhone and iPad Splash Screen Shot.: The splash screen of the iPhone app. It
shows the OMB#, expiration date and the Burden statement. Burden statement
and privacy notice are also accessible from the Info link on other app pages.




iPhone App About Screen: This file shows the “About” screen of the iPhone
app. Shows relevant logos.

iPhone Screen Shot Burden Statement: Shows the burden statement in larger
font. The text is scrollable and can be emailed for review.



iPhone User Privacy Screen Shot: This page shows the user notice that is also
available to the Emergency worker at any time through the information and
About pages/ links.



iPad Screen Shot About, Burden Statement, and privacy statement: This
screen combines the above three screens into one, taking advantage of the
larger screen real state of the iPad.


iPhone/iPad Screen for Data Entry for Emergency Worker: This screen is
shown to the Emergency worker when entering relevant information about the
found person.

iPhone/iPad Screen Shot with collected info for PEOPLE LOCATOR Web
site: This information shows a summary of all information entered by the
Emergency worker for a found person. This information is then sent to the
Reunification Web site that is accessible by Family members.





2. For Family Members: All files in this folder contain screen shots of the Web site
that is intended for searching by family members or loved ones to locate any found
people reported by using the iPhone app. The folder contains the following files
Site presented to Family Members Searching for Loved One#1: This is the
Web site that is first presented to the user. In this case the “PEOPLE LOCATOR for
the 2012 Philippine Floods” site. It will be similarly customized for other specific
future disasters as they occur. The site shows the OMB statement at the bottom:

“OMB NO: 0925-0612 EXPIRATION DATE: 6/30/2013”. When clicked/selected it shows
the full text in a pop-up box. See Site presented to Family Members Searching for

Details of Found or Missing Person on Site: This shot shows results from a
search done for a Found or missing person by the name “Brown”.

Family Member Searching for Brown: This page shows the detail view of the
information in the database that is reported by the Web site when a user clicks
on a picture of a found or missing loved one.
