Circumstances of Change Request for OMB 0920-0134

To enable electronic submission, CDC is proposing a nonmaterial/non-substantive change to the approved information collection OMB Control Number 0920-0134: Foreign Quarantine Regulations (expiring 05/31/2019).

The proposed change to the information collection involves a clarification of terminology regarding the application process to import dogs that may not have been adequately immunized against rabies. The primary change involved a change from the term “permission” to “permit,” to clarify that what is being granted is a formal CDC permit.

CDC is not proposing any changes to other parts of the information collection approved in 0920-0134 Foreign Quarantine Regulations. There is not expected to be any impact on the number of burden hours.

Description of Changes

CDC is requesting four changes to the Application for a Permit to Import A Dog Inadequately Immunized Against Rabies (Attachment A):

1) CDC is seeking a change in the title of the IC pertaining to dog importation permit applications. The current title is “Application for Permission to Import A Dog Inadequately Immunized Against Rabies.” CDC seeks a change to “Application for a Permit to Import A Dog Inadequately Immunized Against Rabies” on the form itself and in the guidelines/instructions for the form. Similarly, references to “permission” on the form and guidance document will be changed to “a permit.” This change reflects the fact that CDC issues a formal permit for dog importations, rather than a letter of permission.

2) CDC is seeking changes to support electronic completion of the Application for a Permit to Import A Dog Inadequately Immunized Against Rabies. CDC would like to revise the instructions at the top of the form to highlight the option to submit the form electronically. CDC is seeking a change to the signature bock in Section G to allow for electronic signature of the form.

3) CDC is seeking a change to block 28 to allow importers to upload or attach a photo of the dog. This change will help better identify the dog at the time of arrival.

4) CDC is seeking an additional field in block 39 of the dog importation permit application. This field would allow respondents to indicate an applicable airway bill (AWB) number, for dogs transported via cargo.

CDC is requesting a number of changes to the Guidance Document for Completing the Application for Permission a Permit to Import a Dog Inadequately Immunized Against Rabies (Attachment B) to align with the new Application and to facilitate electronic submission of the document to CDC.

1) CDC seeks to change references from “permission” to “permit” in the guidance document. This change reflects the fact that CDC issues a formal permit for dog importations, rather than a letter of permission.

2) CDC is seeking an update to the reference to the Animal and Plant Health Inspection Service (APHIS) website on the guidance document for the permit application. This change reflects updates to the APHIS website, and directs respondents to the most current information.

3) In the block 32 section of the guidance, CDC is requesting a change to use more plain language in describing when the dog will enter the United States. This is to facilitate CDC’s timely review and processing of the permit.

4) Block 39 will include an instruction for the airway bill, to align with the new data field in the Application.

5) In block 43, CDC is requesting the removal of redundant text asking for the an explanation of why a dog cannot meet the vaccination requirements

6) In block 44, CDC requests clarifying language to indicate that a typed name constitutes a legal signature. CDC’s intent is to make it easier for the public to file this application electronically.

The proposed changes will make it easier for respondents to submit requests for dog importations permits electronically.

Burden

As a result of this proposed update, CDC does not anticipate any change in burden to this information collection.

Privacy Impact Assessment

These proposed updates to the dog import application will have no effect on the Privacy Impact Assessment provided in the supporting statement for the information collection approved under OMB 0920-0134.
