Instructions for the Survey and Consent
This research project seeks your response to several aspects of policing. This survey should take less
than 30 minutes to complete. While completing this survey will provide no direct benefit for you, it will
provide data to be analyzed to support the ability to develop a greater understanding of police
supervisors’ attitudes and opinions and improve policing in general.
Please complete the provided survey and return it to the researchers. Your participation in the survey is
completely voluntary. If you do not want to complete this survey, you don’t have to. If you start to
complete the survey, you can change your mind at any time. Also, you do not have to answer any
questions that you don’t want to answer. Returning a survey indicates your consent to participate in
this study. Your decision to participate in this survey or to abstain from completing the survey will not
be shared with the National Academy or your police department.
All responses will be anonymous. There is no risk to you. No one will know you participated in the
survey, and no one can be identified from the answers that you provide. Do not include your name,
badge number, or any other identifying information on the survey. In addition, your survey answers will
not under any conditions be given to your police department or the National Academy. Your
participation in this inquiry in no way affects your participation in the National Academy. Your
information is for research purposes only and will not be used against you for any civil, criminal, or
administrative purpose.
Participating in this survey is considered human subject research, FBI human subject research
regulations grant you the right to insist that a signed record of your informed consent be retained by the
researchers. If you elect this option, please contact the FBI Institutional Review Board (IRB)
Administrator or FBI IRB Chair and reference study 386-16. You can contact the FBI IRB on this or any
