Other Changes/Justification Statement: This form has changed. The updated form has 15 new sub-categories added to the 3 existing categories: (1) Hispanic, (2) Asian, and (3) Native Hawaiian or other Pacific Islander. Traditionally, the form is offered to applicants at move-in. This new form will be offered to applicants at move-in and tenants at recertification. We will require owners to offer this form for 1 year at recertification and move-in, so that we can get the greatest number of respondents. Then we will revert to offering the form at move-in only.
