Instructions for Teachers

BEFORE DATA COLLECTION

• The school liaison will visit your classroom on the Monday before data collection week to deliver student packets to you and give a brief overview to the class (should take 10-15 minutes)

How you can help with the Student Packets:

• Please write each child's name on the top half sheet

• Write the deadline for your students to return the packets if the schools liaison has not already done so

• Encourage your students to return the packets as soon as possible. Only students with signed parent consent forms may participate.

• Send home one packet per child and request they return the entire packet.

• When packets are returned, please confirm that the parent consent form (purple) has been signed

• Stack packets so that those with signed consent forms are on top

DAY OF DISTRIBUTING ACCELEROMETERS

• At the very start of class, the liaison will read the child assent form to the students as a group

• The liaison will then call up students one-by-one to fit them with belts and confirm their assent

Note: We recommend having a classroom activity occurring simultaneously during belt fitting to limit disruption

DAY OF COLLECTING ACCELEROMETERS

• Remind students to return accelerometers and belts/clips by [DATE].

• When accelerometers are turned in make sure the device, belt/belt clip and accelerometry log is included in the envelope.

• Notify the school liaison of late returns so a text or email reminder can be sent to the parent.

PE TEACHER DATA COLLECTION

• The evaluation team at ICF will send you a link to a survey about PE, physical activity and the PYFP program that should take about 25 minutes to complete

• 6^th grade PE teachers in select schools will also be recruited to participate in focus groups (PYFP schools only)
