You are invited to participate in this survey designed for environmental health (EH) professionals
working at public health departments. The primary goal of this survey is to identify and describe EH
professional characteristics, areas of practice, and identify workforce and practice needs.
The survey should take about 30 minutes to complete. Your participation is voluntary and you may
decide to quit the survey at any time. If you do not want to participate in this survey, please click
the appropriate exit button on the next page to opt out. You will not be emailed again.
All information collected in this survey will be aggregated for analysis. This means that it is
impossible to identify any individual or organization, or to link you and your responses. No personal
information gathered in this survey will be shared with any other organization. By submitting the
survey, you agree to participate in this this study and agree to the use of your responses as outlined
above.

UNCOVER EH Survey
Purpose

Understanding the Needs, Challenges, Opportunities, Vision and Emerging Roles in Environmental
Health (UNCOVER EH)
The National Environmental Health Association, Baylor University, and Centers for Disease Control
and Prevention’s National Center for Environmental Health are partnering on this groundbreaking
initiative to describe and assess State, Tribal, Local and Territorial (STLT) environmental health (EH)
professionals and their scope of work. This collaborative effort will also identify current and future
challenges impacting the EH practice. In order to accomplish these goals, we are surveying EH
professionals working in STLT governmental public health departments to identify, describe, and
understand:
  • Grand challenges and emerging environmental health issues;
  • Workforce demographics, skills and competencies, and potential training needs; and
  • Services, activities, primary responsibilities, and delivery of the 10 Essential Environmental Public
Health Services.
The results of this initiative will provide important information about workforce that could contribute
to activities such as:
  • Establishing uniform practice benchmarks and workforce competencies;
  • Allocate funds and resources to support improvement of the practice based on identified needs; 
  • Informing decisions regarding services and activities; and
  • Supporting broader public health initiatives, such as workforce enumeration, health department
accreditation, and academic research and training efforts.

In this part of this survey, we ask your opinions on which workforce and professional attributes you
believe are most important.
The following section contains a set of eight comparisons, each comprised of subsets from a list of
eleven possible priorities.
Why do we ask you to complete eight comparisons? With this information we can prioritize workforce
and professional attributes for each individual taking this survey. With priorities for each individual,
we can then aggregate and compare groups (i.e., how professional attribute priorities vary
geographically or among different professional organizations).
This section takes some thought and patience. After the first comparison or two, the balance will be
much easier.
After you finish this section, we will show you your personal ranking of the workforce and profession
