Introduction

This survey is being conducted by the Bureau of Labor Statistics Census of Fatal
Occupational Injuries Program (CFOI).
The purpose of this brief survey is to find out more about how you use CFOI data so that
we can continue to provide you with the highest quality data possible.
We hope you will be able to help us identify areas where we can improve our products and
customer service. We will combine your answers with others responses to produce
summaries where individuals cannot be identified.
