Responses given are kept confidential and are retained only for statistical purposes. Response is voluntary and
not required by law. However, your report is needed to make regional and national estimates as accurate as
possible.

According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person is not required to

For steps 10 and 11, you have the choice of one of two methods to select sites randomly. The first

10a. Visit the www.Random.Org website. See Figure 1 for an illustration of the website and steps i-iii below.
10b. In order to generate random numbers, follow steps i-v below.
i.
In the “Max” field enter the (question 6) number of sites in this [State].

Press the “Generate” button to produce a single random number.
iii.
Record the random number that is displayed in the “Random Number Generated” column of the
table in (question 12) below.
iv.
Repeat steps ii and iii until (question 9) sites are chosen.
v.
If a randomly-generated number is the same as a previously-generated number, discard that
number, it will not count towards reaching the number of (question 9) sites. Be sure to replace
this discarded number with a valid number using steps ii and iii.

Choosing random numbers using a random number table (use if no internet connection is available)
Note – if you have difficulty with using this table, you can call NAHMS at (866) 907–8190 Monday-Friday,
6am – 6pm for assistance.
11a. See Figure 2 for a random number table that will be used to select a random number of sites.
11b. To choose random numbers from the table, perform steps i-iv below.

Write the day of your own birthday (e.g., if your birthday is January 10th, you would write “10”

DIRECTIONS: Start in row 1 and your columns from step ii. If the number in this position is valid
(i.e., between 1 and the total number of sites from (question 6)), then write it down in the
“Random Number Generated” column of the table in (question 12). If that number is not valid, do
not write it down in the table in (question 12). Then, proceed to row 2, row 3, row 4, and so on,
and write valid random numbers in the “Random Number Generated” column until the appropriate
number of sites (question 9) has been reached

iv.

EXAMPLE: If we have 45 sites and are looking in columns 10 and 11, we would start in row 1
and write down “25”, then “14” from row 2, then skip “63” and “99” in rows 3 and 4, since 63 and
99 are both above the total number of sites and continue similarly until the number of (question 9)
sites are chosen.
NOTE: if you reach row 30 and have not met the number of (question 9) sites, continue in row 1,
using the next set of columns to the right of the set just used (e.g., if we reached row 30 using
columns 10 and 11 and still needed to select more sites, we would start again in row 1 of columns
12 and 13).
If a randomly-generated number is the same as a previously-generated number, discard that
number, it will not count towards reaching the appropriate number of (question 9) sites. Be sure to
replace this discarded number with a valid number using step iii.

12. If (question 3) = No or (question 6) = 1 or 2 sites, write the site names and contact information for those sites
