Unemployment Insurance Supplemental Budget Request Activities

NON-SUBSTANTIVE CHANGE REQUEST: The U.S. Department of Labor (DOL) Employment and Training Administration (ETA) Office of Unemployment Insurance (OUI) requests OMB to authorize a form number change to Information Collection Request (ICR) for ETA Form 9165-Quarterly Narrative Report form and instructions, to ETA Form 9178 (OMB Control No. 1205-0517). No other changes to the form are being requested, only the form number.

The ETA 9178 report contains information on activities funded by the US Department of Labor’s (Department’s) Employment and Training Administration (ETA) through Unemployment Insurance (UI) Supplemental Budget Requests (SBRs), including the funded project/activity, the targeted start and completion dates for the project/activity, and the quarterly implementation status. These data are needed for budget preparation and control; program planning and evaluation; program monitoring, oversight, and performance accountability; actuarial and program research; and for accounting to Congress and the public. This collection is authorized under the Social Security Act, Title III, Section 303(a)(6). The information to be collected on the ETA 9178 report will be used by the National and regional offices to monitor the progress of State Workforce Agencies (SWAs) in successfully implementing projects funded through SBRs.

There was an inadvertent incorrect issuance of the next in line to be used form number, by former ETA PRA Forms Manager. This non-substantive change request is to amend Quarterly Narrative Report form and instructions for OMB control number 1205-0517 Unemployment Insurance Supplemental Budget Request Activities, to display correct ETA Form 9178 number.
