The contractor, John Snow, Inc. (JSI), and the project consultant subcontractors are partners in this effort and have worked jointly on the design of the project.


Contract Staff:


The key personnel from JSI who have led the design of the project include:


  • James Maxwell, PhD, Principal Investigator, Director of Research and Policy at JSI


  • Angel Bourgoin, PhD, Project Manager, Researcher at JSI


Consultants:


There are five consultant subcontractors on the project who have collaborated with JSI on the development of the data collection tools and protocols.


  • Amy Boutwell, MD, MPP, Co-Principal Investigator, President of Collaborative Healthcare Strategies


  • Richard Balaban, MD, Co-Principal Investigator, Medical Director of the Department of Care Integration at Cambridge Health Alliance (CHA)


  • Jeffrey Greenwald, MD, SFHM, Investigator, Massachusetts General Hospital


  • Ted Palen, MD, PhD, MSPH, Investigator, Kaiser Permanente Colorado (KPCO)


  • Michael Hochman, MD, MPH, Investigator, University of Southern California


In addition to their roles in advising the project, Dr. Balaban will lead the data collection at Cambridge Health Alliance, Dr. Palen will lead the data collection at Kaiser Permanente Colorado, and Dr. Hochman will lead the data collection at AltaMed.


Technical Expert Panel (TEP)



JSI established a Technical Expert Panel (TEP) to provide expert advice on how to develop a REV to better deliver care to recently discharged hospital patients. The TEP will provide guidance at several points throughout the project, such that they will be able to offer input on all the major tasks contributing to the development of the REV: the Environmental Scan, Analysis of Primary Care Processes, Identification of REV components, and dissemination of the project’s deliverables and findings. The TEP is made up of ten individuals:

  • Thomas Bodenheimer, MD, MPH, Director of the Center for Excellence in Primary Care at the University of California San Francisco


  • Henry Chung, MD, Vice President and Chief Medical Officer of Montefiore Care Management


  • Lisa Latourneau, MD, MPH, FACP, Executive Director of Maine Quality Counts


  • Carol Levine, MA, Director of the United Hospital Fund's Families and Health Care Project, Founder of Next Step in Care


  • Shawna J. Perry, MD, FACEP, Associate Professor, Department of Emergency Medicine, University of Florida


  • Walker Rosenberg, MSW, Transitional Care at Rush Medical Center, Project Bridge


  • Kevin Schuer, DrPh, PA-C, University of Kentucky College of Health Science


  • Gail Sanchez, PharmD, BCPS, Pharmacist involved with Project RED at Boston Medical Center


  • Bernadette Thomas, APRN, Chief Operating Officer, Lynn Community Health Center
