Short Statement: Participation in the survey will be on a voluntary basis and agencies will be able to withdraw at any time. In designing the assessment, questions were carefully considered and efforts were made to avoid redundancies. The survey will be administered online and should take no more than twenty (20) minutes to complete. Only one designee per agency will be asked to fill out the survey.
