This data collection provides instructions and forms necessary for States to report the number of written, signed complaints; mediation requests; and hearing requests and the status of these actions with regards to children served under Part C of Individuals with Disabilities Education Act (IDEA) initiated during the reporting year. The form satisfies reporting requirements and is used by OSEP to monitor SEAs and for Congressional reporting.

There is an adjustment decrease of -1120 annual burden hours. The estimated reported burden has decreased due to stabilization of the data collection, the continued use of an electronic information collection system that give states immediate feedback on errors, and the small number of states required to resubmit their data.
