Study Description

As you know, your adolescent was selected to take part in an important study for the U.S.
Department of Health and Human Services called the National Mental Health Study (NMHS).
The NHMS is a nationally representative study that will:

Examine the frequency of a variety of mental health conditions;
Investigate connections between mental health problems and treatment.

Knowledge gained from this study will improve our ability to describe and understand mental
health issues in the United States. This study is sponsored by the Substance Abuse and Mental
Health Services Administration (SAMHSA) and the National Institute of Mental Health (NIMH),
both part of the U.S. Department of Health and Human Services (DHHS).
We would like you to take part in the NMHS Parent Interview to answer questions about your
adolescent’s health, development and family background. A parent’s perspective is very
important in getting an accurate description of an adolescent’s health and development. The
interview can be conducted via the web or over the phone and takes, on average, 30 minutes
to complete. Your participation is voluntary. If you agree to complete the interview, you
will receive $30 today.
While the interview has some personal questions, federal law protects the privacy of your
answers and requires us to keep all of your answers confidential. Any data that you provide
will only be used by authorized personnel for statistical purposes according to the Confidential
Information Protection and Statistical Efficiency Act of 2002.

The National Mental Health Study (NMHS) Parent Interview, sponsored by the Department of
Health and Human Services, asks questions about your adolescent’s health, development and
family background. The NMHS is a nationally representative study that will examine the
frequency of a variety of mental health conditions and investigate connections between mental
health problems and treatment. A parent’s perspective is very important in getting an accurate
description of an adolescent’s health and development, which is why you have been selected to
participate.
This interview will take approximately 30 minutes. You should answer the interview questions in
a private setting.
While the interview has some personal questions, federal law keeps your answers private. Any
data that you provide will only be used by authorized personnel for statistical purposes
according to the Confidential Information Protection and Statistical Efficiency Act of 2002.
Your participation is voluntary. You may consider some of the questions to be sensitive in
nature and some of the questions may also make you feel certain emotions, such as sadness.
Remember that you can refuse to answer any questions that you do not want to answer, and
you can stop the interview at any time. If you become upset at any time during the interview
and wish to speak to a mental health professional about how you are feeling, you have toll-free
hotline numbers that are printed on your payment receipt you received from the interviewer.
These study details are also included on the Study Description you received from the
interviewer who met with you in your home. If you have about the study, call the Project
