Revision to the Collection Instrument
Change #1: Add a form letter to be included with the SSA-521 when we receive a request to withdraw an application at the hearing level.
Justification #1: We are adding this letter, as we believe it will increase the likelihood that recipients will return the form. In addition, this cover letter provides a way for the recipients to contact the office if they have any questions.
This action will not affect the public reporting burden.
