Form SSA 521, Request for Withdrawal of Application, allows claimants to specify which application they want to withdraw and the reason for the withdrawal. Form SSA-521 is our preferred instrument for a withdrawal request; however, any written request for withdrawal signed by the claimant or a proper applicant on the claimantâs behalf will suffice. Individuals who wish to withdraw their applications for benefits complete Form SSA-521 or sign the completed form for each request to withdraw. SSA uses the information from Form SSA 521 to process the request for withdrawal. The respondents are applicants for Retirement, Survivors, Disability, and Health Insurance benefits.

When we cleared this ICR in 2023, the burden was 6,261 hours. However, we are currently reporting a burden of 6,824 hours. The increase in burden is due to increase in the number of responses from 75,127 to 81,891. These figures represent current Management Information data.
*Note: The total burden reflected in ROCIS is 81,891, while the burden cited in #12 of the Supporting Statement is 6,824. This discrepancy is because the ROCIS burden reflects the following components: field office waiting time + a rough estimate of a 30-minute, one-way, travel burden + learning costs. In contrast, the chart in #12 above reflects actual burden.
