T ITLE OF INFORMATION COLLECTION: Medical Foster Home (MFH) Program Caregiver Survey


PURPOSE: MFH is an established VHA program wherein a community caregiver is pre-determined to qualify to serve Veterans in their own private home, with the ongoing support of a VHA interdisciplinary home care team. Presently there are 1,000 such MFH Veterans served by 700 MFH caregivers. The survey purpose is to annually gather feedback from enrolled MFH caregivers, and share the survey results with enrolled Veterans, existing caregivers, and internally within VHA. No statistical rigor is planned, nor research, nor dispersal of information with external media services.



DESCRIPTION OF RESPONDENTS: MFH Veterans are community-based Veterans who otherwise are at risk of nursing home admission, if not for the VA-approved MFH caregiver providing the needed care for them in a safe home setting. Thus, the respondents are the MFH caregivers.

VACO does not have such a list. Rather, each of the 121 VA facilities with MFH programs have their own MFH Caregiver list. MFH Caregivers are surveyed locally at each of the 121 participating programs and the anonymous results are compiled on a national SharePoint, minus any PI.

TITLE OF INFORMATION COLLECTION: Provide the name of the collection that is the subject of the request. (e.g. Comment card for soliciting feedback on xxxx)


PURPOSE: Provide a brief description of the purpose of this collection and how it will be used. If this is part of a larger study or effort, please include this in your explanation.


DESCRIPTION OF RESPONDENTS: Provide a brief description of the targeted group or groups for this collection of information. These groups must have experience with the program.


TYPE OF COLLECTION: Check one box. If you are requesting approval of other instruments under the generic, you must complete a form for each instrument.


CERTIFICATION: Please read the certification carefully. If you incorrectly certify, the collection will be returned as improperly submitted or it will be disapproved.
