T ITLE OF INFORMATION COLLECTION: MyVA Access Veteran Interviews


PURPOSE:

The MyVA Access qualitative evaluation project is seeking to understand how Veterans experiences receiving care from the VA has been impacted by the MyVA Access initiatives. This data will be used to inform improvements made to access for Veterans in the VA. This is part of the larger Office of Rural Health funded MyVA Access Evaluation project.


DESCRIPTION OF RESPONDENTS:

We will interview Veterans who have received care associated with 8 VA facilities in 2017.

We will speak with up to 50 Veterans that have received care from 8 of the 22 VA sites that the qualitative team has visited to collect site visit data around the MyVA Access initiative: 4 high performing and 4 low performing sites based on ratings from qualitative data collection templates the project used. Veteran variables will include:

-Volume of care usage over 2107: we will pick 6 high (top third of site utilizers) and 2 low (lowest third of care utilizers) at each site.

-Mental Health services over 2017: we will include at least 3 Veterans from each site who have utilized mental health services.

-Care at CBOC vs. VAMC: the team will speak with at least 4 Veterans that accessed care at a CBOC.

-Specialty care received at another VAMC: the team will speak with at least some Veterans that were referred to another VAMC for their specialty care.

-Gender: the team would like to speak with at least 5-6 females out of the entire sample.

TITLE OF INFORMATION COLLECTION: Provide the name of the collection that is the subject of the request. (e.g. Comment card for soliciting feedback on xxxx)


PURPOSE: Provide a brief description of the purpose of this collection and how it will be used. If this is part of a larger study or effort, please include this in your explanation.


DESCRIPTION OF RESPONDENTS: Provide a brief description of the targeted group or groups for this collection of information. These groups must have experience with the program.


TYPE OF COLLECTION: Check one box. If you are requesting approval of other instruments under the generic, you must complete a form for each instrument.


CERTIFICATION: Please read the certification carefully. If you incorrectly certify, the collection will be returned as improperly submitted or it will be disapproved.


Personally Identifiable Information: Provide answers to the questions. Note: Agencies should only collect PII to the extent necessary, and they should only retain PII for the period of time that is necessary to achieve a specific objective.
