Dear Blue Star Museum,
We hope you and your loved ones are healthy and safe during this time of uncertainty as the global
impact of COVID -19 continues to unfold. Thank you for registering for the 2020 Blue Star Museums
program.
The National Endowment for the Arts, Blue Star Families, and the Department of Defense value our
collaboration with you and applaud your commitment to growing your community to include military
service personnel and their families.
We recognize that in-person programming for many institutions is on hold for the moment. It is our
hope that finding our way through this together will make us all stronger in ways we never imagined.
We know that the arts provide comfort, resilience, wisdom, and the means for self-expression and
connection, perhaps even more so during challenging times such as these. We want to support the work
you are doing to provide that connection, education, and relief to the communities you serve.
We are exploring the postponement of the launch of Blue Star Museums from May 16, 2020, to July 1,
2020, and want to ask you a few questions. Please follow the link to a brief Survey Monkey

SURVEY FOR BLUE STAR MUSEUMS OF ONLINE ACTIVITIES
Thank you for registering for the 2020 Blue Star Museums program. We recognize that in-person
programming for many institutions is on hold for the moment. We are exploring the postponement of
the launch of Blue Star Museums from May 16, 2020, to July 1, 2020, and want to ask you a few
questions. Please complete this survey and submit by May 1, 2020.
Your participation is voluntary. You will not receive any compensation for responding to the survey. You
may decline to answer any questions you wish. Under the Paperwork Reduction Act of 1995, no persons
are required to respond to a collection of information unless such collection displays a valid Office of
Management and Budget (OMB) control number. The OMB control number for this survey is OMB No.
3135-0130.
The time required to provide responses to the survey questions is estimated to be approximately 5
