I am contacting you on behalf of the National Association of County & City Health Officials (NACCHO) to assess the status of vector surveillance and control programs in your district or jurisdiction. NACCHO, in partnership with the Centers for Disease Control and Prevention (CDC), is administering a brief electronic assessment to local vector control districts and health departments.
The goal of this assessment is to understand the status and needs of your district or jurisdiction regarding vector control and surveillance. While your participation is voluntary, and not compensated, your response is essential to document the current status of vector control activities and competencies across the country. The CDC and NACCHO will use this information in de-identified, aggregate form to best determine how to support vector control and surveillance activities in response to vector-borne disease threats.
This brief questionnaire should take no longer than 9 minutes to complete.
Please respond to this assessment by Day of Week, Month, Date, 2020.
Instructions:
This is not a blind assessment. This assessment is unique to <Vector Control Program/Organization/District>.
Each organization will receive a ranking of “Fully Capable,” “Competent,” “Needs Improvement,” or “Cannot Assess” for mosquito control capabilities. The aggregate competency statistics will be released, however, identifiable information about each organization’s competencies will only be shared with members of the assessment team.
